Are Neanderthals just a mix of Denisovan and early modern humans? 

Then you have stats like this. Denisovans and Neanderthals obviously form a huge clade, and if Eurasians have Neanderthal ancestry, they should also have a slight connection with Denisovan relative to Africans, but instead it's the opposite?

Do modern Eurasians (by and large) actually have Neanderthal ancestry? Or is the connection between them just that something that Nesher Ramla contributed to both?
